Diseases and Immunity
Every question in this chapter is really asking the same underlying thing in different clothes: a pathogen has to get from one host to another, and the body — sometimes with help from clean water and good hygiene, sometimes with its own antibodies — is trying to stop that from happening. Once you can see the chapter that way, most of the 'which topic is this question actually about' confusion disappears.
Cambridge groups all of this under a single syllabus point, 10.1 Diseases and immunity, but it rewards thinking about it in stages. Core candidates need to know what a pathogen is, how it travels between hosts, what the body does to keep it out, and how public health measures interrupt that journey before it even starts. Supplement candidates go one step further, into how antibodies recognise pathogens with near-lock-and-key precision, how vaccination gets that protection in place before you ever meet the real thing, how passive immunity differs from active immunity, and how cholera turns a straightforward water-treatment problem into a genuinely dangerous osmosis problem inside the gut.
Students tend to lose marks here less because they don’t know the biology, and more because they answer at the wrong level of precision. “Wash your hands to stop germs” states an idea an examiner already agrees with, but it doesn’t say which route is interrupted or how — and that’s usually where the mark actually sits. The pages below are written to close that specific gap: not more facts, but the exact phrasing and causal chains a Cambridge mark scheme is built around.

Core or Supplement?
The first three pages — pathogens and transmission, body defences, and controlling the spread of disease — are Core, so every candidate needs them regardless of tier. The remaining four — antigens and antibodies, vaccination, passive immunity, and cholera — are Supplement only, required for the Extended paper, not for Core-only candidates. If you’re not sure which tier you’re sitting, that’s worth checking before you spend time on the Supplement pages, since none of the Core content depends on them.
