Pathogens and Disease Transmission
Two definitions do almost all the work in this section, and they get mixed up more often than you’d expect: a pathogen is what causes a disease, and a transmissible disease is one whose pathogen can be passed from one host to another. Confuse the two and an exam answer starts on the wrong foot before it’s even reached the interesting part.
A pathogen is a disease-causing organism. That’s the whole definition, and it’s worth holding onto exactly as it is rather than reaching for a synonym under exam pressure. A transmissible diseaseis a disease in which the pathogen can be passed from one host to another — so the word “transmissible” is describing the disease, not the organism, even though in practice students often use the two almost interchangeably. Keep them apart: one names the cause, the other names a property of the illness that follows from it.
For a transmissible disease to actually spread, the pathogen has to reach a new host somehow, and the route it takes is where most of the exam marks in this section live.
Direct transmission
Direct transmission means the pathogen passes straight from one host to another, with nothing in between. Cambridge specifically wants you to know that this includes transmission through blood and other body fluids— a detail that comes up more often in unfamiliar-context questions than students expect, because it’s easy to think of “direct contact” only as touch, when the syllabus is really pointing at fluid exchange.
Indirect transmission
Indirect transmission means something sits between the infected host and the next one, carrying the pathogen across. Cambridge lists four routes you should be able to name specifically: contaminated surfaces, contaminated food, animals, and air. A pathogen picked up from a doorknob, from undercooked food, from a mosquito bite, or from someone coughing nearby are all indirect — the common thread is that the pathogen survives outside a host for at least a moment before reaching the next one.
Why the route matters more than it looks
The distinction isn’t just classification for its own sake — it’s the reason different diseases need completely different control strategies. A measure aimed at stopping contaminated food from being eaten does very little against a pathogen that mostly spreads through the air, and vice versa. This is exactly why controlling the spread of diseaseis treated as its own section rather than a single blanket answer like “be hygienic” — each control measure is built to interrupt one specific route, and a strong exam answer says which one.
In practice, this means a complete answer names the route (direct via body fluids; indirect via contaminated food, water, surfaces, air, or an animal) rather than stopping at “it spreads between people.” If a question gives you an unfamiliar disease and asks how it spreads, look for the clue that tells you which category it falls into — waterborne outbreaks and contaminated-food outbreaks are indirect; contact with blood or another body fluid is direct — and build your answer around that specific route rather than a general description of infection.
